  $500 or more / None / None
  $300.00 to $499.99 / $2.50 / 20 cents
  $200.00 to $299.00 / $3.75 / 20 cents
  $100.00 to $199.99 / $5.00 / 20 cents
  $99.00 or less / $6.00 / 20 cents
